The custom forms are great tool for operators to captured key events and information that may not be otherwise captured by the other logs in the app.
They can be completed by:

(I.E. passengers or contractors etc) on their own devices.
Multiple users can collaborate on the same form using the powerful Save for Later function, allowing partially complete logs to be saved for completion at a later date.
These will appear in a list when the users selects the type of log they would like to complete, and they can either choose to start a new log, or select from the list of current Open logs.
Admins can nominate one or more email addresses to automatically receive a summary of any completed forms.
Available Field types:
Text Box:
An open text field for any text or numbers.
! Input limited to 100 characters.
Paragraph:
An open text box for any text or numbers.
! Unlimited character input.
Drop down Box:
A list that users can select ONLY ONE OPTION.
Checkboxes:
A list that users can select MULTIPLE OPTIONS.
Doodle Pad:
Most often used as a signature field.
Location Pin:
Captures the devices location data to produce a Lat and Long (the user can choose either decimal or degrees minutes and seconds), as well as What3Words location (useful for emergency services and particularly for non-vessel assets):

Date picker:
Allows the user to select a date AND TIME using the scroll wheels.

Photo Upload:
Allows users upload an image, either from their devices photo library or take a new image to include.
Read Only Text or View Only Picture:
Text or Images that app users can only view.
Crew picker:
Allows users to select multiple options from the list of users who are assigned a role on the asset.
Note: this field must be included in the form if you want to link it to a Crew training schedule (Enterprise feature).
Risk Matrix:
Allows the user to allocate a Likelihood and Consequence rating, and automatically records the calculated risk score.
