Create and Edit a custom form
How to build and manage your custom form.
Create a New Custom form
From the Assets home page, select the Custom Forms tab, then select "Add a Custom Form" at the bottom of the page.
You can choose to either create a new asset unique form, or include a link to Fleet Custom Form. (see Fleet Content for more information on fleet level content).
Select "Create a New Custom Form" and give the form a title (and optional description) in the pop-up window, then click "Add".
You will enter the Edit mode for the form:
Edit your Form
Select the form to edit from the list, then select Edit Custom Form.
Select "Add a Field" and choose from the options. (see Key Features and functionsfor information about each field type).
Give the field a label (usually a prompt or question. I.E. "Weather forecast") and some optional prompt text, response options for drip down or checkbox fields, or other required content, then select "Add to Fields".
Once you have multiple fields, you can drag and drop to re-order, select the plus icon on the right to add a field below the current field, or do any of the following:

A checkbox field might be "Select the PPE Required", with some items listed as potential responses as well as the option "Other". If the user selects "Other", a text field will appear, allowing the user to write a description of the other PPE required.
To do this:
On the tile for the field, select "Add a condition" then "Add New Condition" from the pop up window.
Select the field and the response conditions that the dependant field is to rely on.
Add additional conditions (the user can select whether the dependant field will appear if either ANY or ALL of the conditions are met).
Make sure you select Save Conditions.

Make a field mandatory
A custom form cannot be submitted by the user unless all mandatory fields have been answered.
To make a field mandatory, in the field's Edit window, select "Make this Field Mandatory" at the bottom of the window.
A red star indicates the field is Mandatory, and the Field title appears in red text in the app.
Allow multiple responses to a text field
Only Text Box fields have the option to allow multiple responses.
I.E. The field title may be "Passenger Names", and the user could continue to input multiple responses.
To do this, in the Text Box field edit window, select "Text answers can be repeated".
Display multiple fields on the same page
By default, each field will appear in the app on a separate page that the users has to click Next to navigate to.
By indenting the field, it will display below the field above it.
Indenting multiple consecutive fields displays them all under the last field that is not indented.
To do this, in the field tile, select "Indent".
Duplicate a field
From the field tile, select the Duplicate Icon on the right end of the tile.
